Does the name of your key in the .lang file match the name in the CustomSubItemTypeName in the level editor under the entity tab?

For the checkpoint, what you must do is you have to re-apply all the functions that happen when encountering the monster. I will use my script for example What this does is before you die the function...

Did you put the name of the note in the NoteText area under the entity tab in the level editor

Oh! wait I see the problem now, your missing a ">" at the end of both your </Entry. Add the closing brackets to the end of your entries and that should soolve the problem

Try backspacing before the first bracket in your void func_slam (string &in asParent, string &in asChild, int alState) see if that helps and also you don't need the quotes inside those bracket...
