LevelEditor.exe caused an EXCEPTION_ACCESS_VIOLATION in module nvoglv32.DLL at 0023:6958FE32. This pops up when i try to open my custom map in the editor.
1. What operating system do you have? Also 32-bit or 64-bit?
2. Is it a laptop or a desktop? If a laptop, what model?
3. Attach me here the file HPL.log found under Documents\Amnesia\Main folder.
I have a laptop. Windows 7 64-bit version.It's a Dell XPS M1710.
Does this happen every time you open level editor, or just occasionally on some maps?
Try to delete thumbs and temp folders in Documents/HPL2
I have been making a custom map and whenever I try to open that it crashes. Maybe once in a blue moon though it doesn't crash :/

Sadly, that didn't help. It crashed with the same exact message...
Can you post your leveleditor.log under Documents/HPL2 ?

If you are on steam try to verify integrity cache as it seems like some of your entities are broken and when level editor tries to load them, it crashes.

How do i do that?
Right click on the game in your Steam library and click on properties. There should be something like "Verify integrity of game cache".
