I'd certainly be very intrigued if the new setting and story went more into the direction of "gritty realism" instead of "scary fairytale", if you know what I mean
Supernatural elemts have been a core elemt of horror for so long, but mere human cruelty has it's very own brand of horror that I feel is too rarely explored in horror games.
Child workers would be a very nice touch I think. It would paint a much more interesting and ambiguous picture of Oswald as a character, than every cartoonishly evil trait like "omg he's turning people into pig monsters" ever could.
