Okay so I'm in the northern block of the prison, with the glass jar,
But I tested the door a few times and whenever I go near the door the grunt re-spawns
I know the grunt is supposed to be there after entering the kitchen with the jar...

I know for sure that it actually res-spawns and isn't just waiting out there. the growl means he spawned.

But I didn't count on it respawning.

Can anyone help? If not I will have to go with the theory:
If you still have the specific object, glass jar, it will respawn, but if you fill it with acid, it won't respawn, anti-trigger would to remove that object. Maybe?

EDIT: I filled it with acid but it keeps respawning.
Humm I have no technical solution for this problem, but if you just go out there and let him kill you, you'll just respawn in a nearby room and the monster should be gone.
I didn't have a respawn problem, but my grunt just got stuck at the door, spinning around his axis. I simply let him see me which got him unstuck, then dashed past him and hid into the hole I entered the prison by until he buggered away.
I've noticed that if they don't despawn, and then become alerted to your presence again, they make the same howl as they do when first spawning. Perhaps that is what's happening to you?
dont know but care

Does anyone know the script for when the ground should shake and when to add a scream effect inside in game. Thank you!

Huh? If you're saying you don't care, then why the fuck did you bother posting?
Damn, I wish I encountered that bug. That'd give me one hell of an interesting challenge. Could I have your save file?
