Lets not forget the insects! Bugs crawling on the screen and I believe in the later parts of the game you find giant spiders on the walls with low sanity.

Not entirely sure if this is it but I do remember a door being blocked by woodplanks around that area, you have to use a saw to get through. Sorry if thats not it though.
