Try used something other script than "AddEntityCollideCallback"

for example SetEntityCollideCallback

Most likely the GiveSanityDamage effect is preventing it from happening. Some effects overwrite others when they are called. Try putting GiveSanityDamage to false for its effect and see if it works after that.
