I reinstalled a clean copy and used the patch file as you had suggested. Still no monsters.

Where did you get the game from? Steam? Our store?
If you want to run the game from a shortcut (or from within Steam), make sure to rename the new exe "Amnesia.exe" so that's the one that actually runs.
Did you start a new game or load an old save?

I can't reproduce the issue on my end, anyone else having a problem with monsters not appearing?
