You know, a blog might actually be helpful. Each time you notice a change, you write it down, and by the end, you will have a nice journal detailing your trip from start to end.

My opinion, though.
If you find yourself lacking motivation because you'd rather spend time playing videogames, try to incorporate the exercising with gaming. It's certainly helping me (just spent the last hour playing the first Assassin's Creed on 360 while riding an exercise bike).
I really should start going for jogs and lifting a bit... =\

If you're still growing, be careful not to overexcert yourself, especially when it comes to lifting heavy weights. It could permanently damage your body (especially joints, tendrils, bones and your spine...) and even hamper your growth. Just putting that out there... apart from that, good luck with your efforts.

Just take it easy; you don't need to exercise constantly. Minor injuries can easily become permanent ailments if you don't give them time to heal.
