The models which rip the hearts out of their chests are included alongside the big_kid one. They're normal sized, and have their hearts floating next to them (you can see them in the 4th pic, I added them next to the big_kid to show the scale difference).

They're not enemies, but they're listed in the "enemies" category [click].
The kids that we see in-game moving from time to time are actually enemies if I recall correctly. Enemies that follow a certain path from a distance from the player while having disabled triggers. At least that's how it was in the beginning of making the game, someone had said so. Probably afterwards, they found another way (props?) to implement that without tagging those models as enemies.
Um... I'm not sure if I've heard "vo_pigline_2_mandus_machine" in game, check it out guys. I can't remember if it was in the game.

You're right, this is unused (as far as I know, anyway!). Here are a few other voice overs I don't think were used:

The last three are from an alternative ending which was abandoned during development.
I found some sound files in a folder named "Infection" that I don't believe were used in the game.

Also, remember that thing a while ago where thechineseroom asked for peoples cries of terror before the game came out? I wonder if some of the stuff we sent them (including myself) are hiding in some of AMFP's audio files.
These are heard in the end of the game before you push the button.
Actually, most of the screams are heard on the conveyor belt leading up to the temple.

The ending originally included a "Big Ben" sound file, which featured a clock tower sounding off. I suppose this might have been used in an older, more ambiguous ending where Mandus didn't have his ending monologue about hearing "a church bell ringing out".

In sounds\music\temple there's music_credits.ogg which is a woman singing When I Was a Laddy with a simple piano playing.
